Public Health Service Athletics events are led by Commissioned Corps Officers volunteering to promote and record engagement in either events developed solely for PHS Athletics or as a group participating in event put on by other organizations. Before submitting an event request, review the materials below to understand event qualification requirements, participation tracking, and submission procedures.

Before Submitting an Event Request
Review the Leader Cliff Notes and Qualification Criteria before submitting an event request. Submission of an Event Request Form does not guarantee approval. Event qualification is determined based on published PHSA Athletics criteria and the Operations Team evaluation.
